Our Services
Our Occupational Therapist (OT), Angus, will meet with you to complete a functional capacity assessment (FCA). During this Assessment, the OT will ask you about your day to day needs as well as observing you within your home environment to gather the necessary information to write an informed report. This report is then used as evidence to show the NDIS how your needs may have changed, or stayed the same, since the last time they have assessed your funding package. If you are new to the NDIS and need an assessment to determine what funding you need in your plan.
Our Occupational Therapist (OT), Angus, will meet with you to discuss your immediate assistive technology (AT) requirements. This could be a wheelchair, mobility scooter or adaptive recreational equipment that facilitates your participation in chosen hobbies. The OT will then schedule all necessary trials, organise scripting and quotes, and write the supporting documentation to be submitted to the NDIS or TAC.
Our Occupational Therapist (OT), Angus, will work closely with you and a wider support team from start to finish to complete a thorough home and living application with the NDIS. Specialist Disability Accommodation (SDA) is an essential, life-changing support for many NDIS participants. Our OT has experienced success with assisting clients to pursue this in the past and will be supporting you with addressing and documenting the specific criteria necessary for SDA funding.
Our Occupational Therapist (OT), Angus, will work alongside you and your support to help you understand whether Supported Independent Living (SIL) is the right fit to suit your high level of daily support needs. Often provided within a shared support environment, a SIL assessment requires stakeholder liaison between support coordinators, SIL providers and support workers to gather the robust evidence necessary for a successful application.
Our Occupational Therapist (OT), Angus, is both experienced and accredited with NDIS home modifications. He will meet with you at your home to understand and observe the physical barriers you are currently experiencing. The OT will then collaborate with you to develop a Home Modification design that suits your functional needs and personal preferences for your home, whilst ensuring compliance with the relevant Australian Standards and reasonable and necessary criteria of the NDIS. Following the initial planning meeting, the OT will engage with a builder to attend a joint visit with the OT for quoting the building works. The final application will include a detailed home modification assessment report, a scope of works document with diagrams and two builders' quotes.
